A glistening is a small fluid-filled cavity, typically ranging from 0.1 to 1 mm in diameter, that becomes trapped inside a lens blank during the manufacturing or annealing process. Unlike a true bubble of air, a glistening contains residual moisture or other volatile compounds that can migrate and expand under thermal stress, making it a defect rather than a benign void. It appears as a localized bright spot or cloudiness when light passes through the affected region of the lens.

Glistenings are most common in hydrophilic acrylic intraocular lenses (IOLs) used in cataract surgery. They form when water molecules become trapped in the polymer matrix during manufacturing or when moisture enters micro-cracks during storage or implantation. The fluid can expand or contract with temperature changes and fluctuations in aqueous humor osmolality, causing the cavity to grow larger or shift position over time. In silicone and acrylic materials, the mechanism and clinical significance differ; some compositions are far more prone to glistening formation than others.

Clinical and optical consequences

Glistenings scatter light and reduce contrast sensitivity, potentially degrading visual quality even if they do not directly obstruct the optical axis. Patients may report glare, halos, or a subjective sense of haziness. The severity depends on the number, size, and location of glistenings within the lens optic. Most remain stable after the first months post-implantation, but some materials and storage conditions favor continued formation over years. High storage temperatures and exposure to moisture accelerate glistening development in certain IOL batches.

Manufacturers address glistenings through tighter control of annealing cycles, reduced water uptake in polymer formulations, improved packaging to prevent moisture ingress, and in some cases deliberate modification of lens material to trap water in a non-mobile form. Standards such as ISO 11979 specify acceptable limits on glistening density and size, but individual tolerance varies widely among patients and surgeons.
