LACES: the five-point checklist that stops confined space deaths
LACES is a hierarchical safety protocol for work in confined spaces, spaces that are large enough to enter but have limited means of entry and exit, poor ventilation, or a design not intended for continuous occupancy. The acronym stands for lookouts, anchor points, communications, escape routes, and safety zones. It functions as a pre-entry verification system: before any worker enters a tank, silo, vault, or trench, the team must confirm all five elements are in place. LACES is not optional procedure documentation; it is a control sequence that has demonstrably reduced fatalities in industries where confined space entry is routine.
Lookouts are dedicated personnel stationed outside the space who maintain continuous visual and verbal contact with workers inside. A lookout does not perform other duties during entry. Anchor points are fixed structural features that can safely support the weight of a rescue harness and line without failure or shifting; improvised anchoring causes many fatalities that LACES procedures exist to prevent. Anchor points must be load-tested and certified where regulations require it.
Communications means a two-way system that functions reliably in the confined space environment. Radio signals fail inside metal tanks; rope signals or hardwired phones may be necessary. Workers must be able to signal distress or abort at any moment without ambiguity. Escape routes are pre-identified, unobstructed pathways or rescue mechanisms that allow a worker to leave the space quickly under their own power or assisted by others. In emergencies, a single blocked exit has sealed many spaces.
Safety zones are designated areas outside the confined space where workers can gather, where rescue equipment is staged, and where medical treatment can be administered if needed. These zones must be clear of traffic, electrical hazards, and other site operations.
LACES originated in the United States from regulations developed by OSHA and incorporated into the Permit-Required Confined Spaces standard (29 CFR 1910.146). It is now applied internationally and has become the lingua franca of confined space safety teams. The acronym works as a memory aid but also as a workflow; teams physically walk through each element before entry. Failure on any one point means entry does not proceed. The protocol is most critical in industries such as wastewater treatment, petrochemical storage, construction, and manufacturing where confined space entry is frequent enough that complacency becomes a recognized hazard.
