rehab: tactical rest for firefighters in active operations
Rehabilitation, or rehab, is a mandatory rest and recovery period that firefighters take during active fireground operations. A firefighter working in a hot zone or performing strenuous tasks will be rotated out to a designated rehab area, typically 50 to 100 feet from the incident, where they step down from operational duty for 10 to 20 minutes. The purpose is to lower core body temperature, reduce heart rate, rehydrate, and allow muscles to recover before returning to active duty. This is not optional downtime: it is a safety protocol built into incident command structure.
Rehab areas are staffed by dedicated personnel, often paramedics or medical support, who monitor vital signs and oversee recovery. A firefighter entering rehab will sit or recline in a shaded or air-conditioned space, remove heavy protective gear if safe to do so, drink water or electrolyte solutions, and breathe cooler air. Medical staff may check blood pressure, heart rate, and core temperature. The goal is to bring each firefighter back to a safe physiological baseline before returning them to the fireground or sending them out of service entirely.
The rehab rotation is calculated by incident command based on air consumption, work intensity, and ambient conditions. In extreme heat or during interior structural fire attack, firefighters may spend only 15 to 20 minutes in the hot zone before mandatory rehab. Lightweight operations in cooler conditions may allow longer work cycles. The Occupational Safety and Health Administration (OSHA) and the National Fire Protection Association (NFPA) both specify that rehab protocols must be in place whenever conditions present a risk of heat stress or exhaustion.
Firefighter collapse and sudden cardiac events are linked to cumulative heat stress and dehydration during long incidents. A firefighter who ignores fatigue signs can suffer heat exhaustion, electrolyte loss, or cardiac dysrhythmia. Rehab intercepts that cascade by enforcing physiological recovery before symptoms become dangerous. It also protects incident command and the fire department from liability, since rehab decisions are documented and tracked.
Rehab is sometimes resisted by firefighters who want to stay engaged with active suppression work, but incident commanders are trained to enforce it. The term itself comes from the medical use of rehabilitation to restore function after injury or stress; here it applies to temporary restoration of work capacity during a single incident. Effective rehab protocols have measurably reduced fireground injuries and line-of-duty deaths over the past two decades.
